Welcome to The Ottawa Vein & Cosmetic Medical Clinic. For your initial appointment you will be requested to fill out our intake form. You can fill the form out online by simply clicking the link below, or if you prefer you can fill the form out at the clinic before your first appointment. (Please ensure you arrive 5-10 minutes early should you wish to fill out the form at the clinic). On your first visit to our clinic a relevant medical history will be taken and a thorough examination will be done. A comprehensive explanation of the available treatments will be given to you and you will be able to ask as many questions as you wish. The cost of treatment will be also explained to you. The initial consultation will allow us to develop a personalized treatment plan tailored to your schedule and goals. A typical appointment lasts approximately 30 minutes.
We encourage new patients to bring all relevant laboratory reports to their first visit.

*Please note that we require a valid credit card number for all patients in order to secure their booking. All cancellation, changes or no-show fees will be charged in full to the credit card number with which they were held. By scheduling an appointment, you are agreeing to our cancellation policy.
We appreciate your patronage and are committed to providing an excellent experience. Due to the exclusive services that we offer, we have enacted policies to ensure that all clients are given the opportunity to enjoy our services fully. We understand that unforeseen events occur which may need you to cancel an appointment. In order to cancel, we require at least 48 hours advance notice, we can then offer your time to another patient.
For follow-up appointments (as well as Socks measurement appointments) any no-shows, cancellations, or changes within 48-hours of the appointment are subject to a fee of $50.00.
For consultations any no-shows, cancellations, or changes within 48-hours of the appointment are subject to a fee of $75.00.
For Botox/Dysport any no-shows, cancellations, follow-ups or changes within 48-hours of the appointment are subject to a fee of $100.00.
For fillers, PRP, hyperhidrosis, Laser Genesis or IPL any no-shows, cancellations, or changes within 48-hours of the appointment are subject to a fee of $250.00.
For all other treatments not mentioned above any no-shows, cancellations, or changes within 48-hours of the appointment are subject to 50% of the scheduled treatment not to exceed $250.00 per appointment booked.
Please arrive to your appointment at least 10 minutes prior to the scheduled time. Also keep in mind that traffic can be heavy and plan your route accordingly. All services offered have a specific time schedule, and early arrival allows for a relaxed and unhurried experience. If late arrival is inevitable, your service(s) may have to be shortened or rescheduled.
Late Arrival Policy
We make every effort to begin and end appointments on time. If you are late and the treatment cannot start on time, the treatment time will be reduced accordingly. This may mean that not all procedures can be performed. If a client is more than 15 minutes late, the appointment may have to be rescheduled. We regret that late arrivals will not receive an extension of the scheduled appointment time. In special cases, and when our schedule will allow, we may be able to accommodate a partial or full appointment. This will be at our discretion and only with advanced notification of late arrival.
If you are late for a scheduled appointment, our clinic policy offers you two options:
- You may opt to shorten your treatment.
- You may prefer to reschedule your appointment at a more convenient time, however, a cancellation charge will be incurred.
No refunds on services and/or treatments. If you have any concern we would be happy to discuss it with you.
No refunds on open boxes unless the patient developed an allergic reaction to the product.
If you have experienced an allergic reaction to the product that you’ve purchased at our clinic, the item must be returned along with the original receipt within 14 days of the purchase date.
In order to qualify for a full refund, the product must be returned along with the original receipt within 14 days of the purchase date, in its original packaging and gently used. In case of return of the compression stockings, you also need to bring Doctor’s prescription along with the original receipt.
No refunds or credit will be offered after 14 days of the purchase date.
Please note that all products purchased during a promotional event are non-refundable.
